Event forms

To begin the booking process for an event at Our Cabana, please complete and return either the Adult or Girl Event Application, and the Registration Fee Form.

Once your booking is under way, you will receive the following forms in the lead up to your visit.

• Cabana Forms for Adult Adventures
• Cabana Forms for Girl Friendship Events

Our Cabaña Forms

There are three forms which we require each individual to read, complete and bring with them to your event at Our Cabaña: the health form, Sensitive Issues/Photo Release form and one copy of the Our Rights, Our Responsibilities form.

Our Cabaña Health Form

Please make as many copies as necessary and have each individual complete the form in full. This form does not require a doctor's or parent's signature, but can be signed by the individual completing it. You may also need to bring the official health form of your Association. Please be sure to obtain health and accident insurance for your group, as Our Cabaña carries no personal liability, accident or medical insurance for you.

Any food allergies or dietary requirements need to be submitted in advance via e-mail.

If any of the members of your group suffer from a special condition, please make sure that all the details are specified in the health form and there is a list of all the medicines that she is taking.

Please, ask all the members of your group to sign one copy of this form.

Sensitive issues and photo release form

During all of our events at Our Cabaña we will be holding workshops and activities based on the current WAGGGS initiatives. The most current program is called girls worldwide say. Please familiarize yourself with the program via the WAGGGS website: http://www.wagggsworld.org/en/projects/GAT

Some of these workshops will involve discussions on the following issues

• “Together we can end extreme poverty and hunger”
• “Education opens doors for all girls and boys”
• “Empowering girls will change our world”
• “Together we can save children’s  lives”
• “Every mother´s life and health is precious”
• “We can stop the spread of AIDS, malaria and other diseases”
• “We can save our planet” and
• “We can create peace through partnerships”

Please ensure that the parents of all participants are fully aware of the subjects that could be discussed in the safe environment of Our Cabaña with others from different countries around the world.
